Care Planning

The care plans or GP management plans involve a detailed assessment of your current symptoms and treatment followed by identifying ways to improve based on best practice guidelines. Care plans are usually undertaken 6 monthly or annually depending on the severity of your chronic disease.

Team Care

Best care for many chronic conditions is facilitated by a team of health care providers, including specialists, pharmacists and allied health professionals. Your GP plays a pivotal role in coordinating this care to ensure the best possible outcome for you.

Chronic Disease Clinics

For your convenience the surgery runs specific clinic days for patients with chronic disease. The clinic days are designed for you to see your GP to complete the care plan and then visit with a nurse regarding education for diabetes or asthma etc. This allows you to have a comprehensive team based care plan generated on the same day and at the same convenient location.

Health Assessments

health-assessmentHealth assessments are a Medicare initiative aimed at allowing people who may be at risk of illness or who are in a particular age bracket, to have a comprehensive health assessment completed. Health assessments are performed by the practice nurse and your GP. Health assessments are bulk billed so there is no out-of-pocket cost to you. A list of health assessments are below.

Healthy Kids Checks

Healthy Kids Check is for every four year old child in Australia. The Healthy Kids Check will promote early detection of lifestyle risk factors, delayed development and illness, and introduce guidance for healthy lifestyles and early intervention strategies.

45 to 49 Year Old Health Checks

This health check is targeted at people between the age of 45 and 49 years of age (inclusive) who are at risk of developing a chronic disease. A chronic disease or condition includes the following but is not limited to – asthma, diabetes, mental health conditions, arthritis, musculoskeletal conditions, cancer, cardiovascular illness.

Over 75 Years of Age Health Checks

This is an annual health assessment for those in this age group. Comprehensive medical assessment for residents in age care facilities. This is an annual assessment completed onsite at the aged care facility.

Type 2 Diabetes Risk Evaluation

Type 2 diabetes risk assessment evaluation is for people between the age of 40 and 49 with a high risk of developing diabetes. Are you at risk? Please make and appointment to see your GP today.

Mental Health Care Plans

Dr. David Cockram, a psychologist works along with your doctors to provide you with the support and assistance that you need. Talk to your doctor in regards to a referral.


Focused counseling has long been established as an important therapy for many kinds of mental illness. Medicare provides appropriate support to fund dedicated sessions by a psychologist or counselor. Ask you GP to complete a mental health care plan and organise an appropriate referral for you. Mental Health nursing services are being sought.
